Your story can include your real name and photo, or it can be anonymous. Privacy is always your choice.
If you prefer, we can:
Use only your first name or initials

Omit certain details
We will be clear about how we plan to use your story and images. If our plans change, we will let you know. If there is a specific way you do not want your story or photo used, you can indicate that on your release form.
